Scope of Services. Through this Site, Carestick Phlebotomy markets and arranges Mobile Blood Draw, Laboratory Specimen Collection, Specimen Pickup. All work is performed under separate written agreement.
Your Obligations. You agree to be truthful in submissions, secure your access credentials, and use this Site only for lawful purposes.
Prohibited Conduct. You will not: violate any law, disrupt Site operation, gain unauthorized access, harvest data, transmit malware, or impersonate any person or entity.
Copyright. Site content is owned by or licensed to Carestick Phlebotomy and is protected by intellectual property laws. You may not copy, modify, or distribute content without written permission.
Fees. Quoted prices appear in your written service agreement. Payment timing and refund policies are governed by that agreement.
End of Use. Carestick Phlebotomy may terminate your Site access at its discretion. Provisions that by their nature should survive will survive termination.
No Warranties. Carestick Phlebotomy disclaims all warranties, express or implied, including merchantability and fitness for a particular purpose. Site use is at your own risk.
Cap on Damages. Liability of Carestick Phlebotomy arising under these Terms is limited, in total, to the fees paid by you in the prior 12-month period.
Indemnification. You agree to indemnify and hold harmless Carestick Phlebotomy from any claims, damages, or expenses arising out of your misuse of the Site or violation of these Terms.
Applicable Law. These Terms are construed under CA law. The parties consent to venue and jurisdiction in CA for any related dispute.
Updates. Carestick Phlebotomy reserves the right to update these Terms. The current version is always available on this Site, and continued use signifies acceptance.
